Partnership action plan

Your partnership’s activities should be shaped by patient/client need, practitioner capability and local context. Common HJP activities include:

  • referral pathways
  • interdisciplinary training
  • secondary consultation
  • shared care coordination
  • shared monitoring and evaluation
  • learning, reflection and improvement

Use this action plan to agree on what you will do together and who will take the lead. Keep it practical and update it as your partnership develops.
