They have been adapted from forms developed by the Australian Institute of Aboriginal and Torres Strait Islander Studies and the Aboriginal Health and Medical Research Council of NSW and fine-tuned for health justice partnership evaluation. Please make any changes you need to suit your individual place and partnership.
